Hi TicoJPunk, I take Otezla and Cosentyx for palmoplantar pustulosis (PP) psoriasis. After nine months my nail's bumps and pitting disappeared. I am very happy with the results; I can stay on my feet for as long as I want. I work fulltime in a hospital and large clinic campus and although my job is primarily being at a desk, it is not unusual to put in 4-6K steps during an eight-hour shift. I have a history of migraine, so I take another med prophylactically for that. GI issues have waxed and waned, but those too can be attributable to my oral diabetic medication in conjunction with Otezla. I am so much happier and able to get out and about than I could years ago. No one asks me what's wrong with my hands anymore, I don't look like I'm contagious and with intact skin, I'm not worried about picking up an infection. Hand sanitizer no longer feels like a zillion paper cuts every time I use it. I like the safety profile of both medications and not having to undergo periodic testing. I like too that they are targeting specific alleles. There is a new oral just FDA approved: Sotktu, a new non-steroidal topical just FDA approved: VTAMA. Personally, I'll keep using what's in my toolbox. Should I need a different oral in the future if Otezla is no longer being effective, then maybe the new drug will be an option? Don't quit what is working--if you need to re-start it there is no guarantee it will work again. My sister has had plaque for over 20 yrs; she then developed PP about 3 yrs ago. She is on Taltz because she quit her Humira as she was doing so well, then flared, restarted it but it no longer worked for her. Taltz is helping (takes time too), but she'd like to do dual therapy since she has PP and two prior MSRA infections. I delayed my Costentyx dose by five days so I could get my vaccines before taking it. One huge lymph node upper neck swelled 1.5 days, on side of Covid bi-valent booster, Flu side, nothing, no ill effects. Good luck to you and stay well!
