TNF Blockers versus Tremfya or Skyrizi

PsA has caused quite a bit of enthesitis in me. I have mostly used TNF blockers since I was diagnosed 2.5 years ago, but I've recently read research about success with IL-23 blockers and enthesitis. Has anyone experienced noticeable improvement with switching from a TNF blocker to Tremfya or Skyrizi?

    1. I'm interested in learning more from other members about Tremfya or Skyrizi helping with enthesitis. My hands and feet are hurting (wish I could write "hurting" in flashing red!) due to enthesistis. I'd like to flatten my hands between a couple of bricks actually. I am on Cimzia which I believe is a TNF blocker or in that family.
      Any suggestions for enthesitis pain would be appreciated!

      1. Tremfya was no help to me and I am just beginning Skyrizi fingers crossed. I have used a laundry list of biologics which also were unhelpful, enbrel, simponi, remicade, humira, Otezla… on and on. Everyone is different so my experience may be different than yours, hope they find one that works for you… so far I have been SOL 😢

      2. its so true that we all respond differently to various drugs. Your kind response helps me know that I am not alone in playing " musical medication chairs". I've been on 3 biologics and a long list of DMARDS.
        If I am not careful, I can feel like a failure for not responding better to treatment.
        While I am sorry that you are in the PsA boat too, your words were a comfort and encouragement to me!

    2. storsie, I was initially on Humira and then went to Taltz, the Cosentyx…these are the Il-17 blockers. They have worked much better and I also have enthesitis….things aren’t perfect and we are considering adding a second medication, Otezla a Pde4 inhibitor. I’m holding off till I’m sure I need more relief as my experience with Humira and Xeljanz was not good…. I lost weight on Taltz but Cosentyx, an Il-17 inhibitor also has not caused that. As for the Il-23 that is upstream of the Il-17 so I’m assuming there’s some overlap in their action

      1. if you are worried about combining Otezla with another medicine I can understand your concern. All it takes is a bad reaction to another treatment to make us question if we really want to chance adding two treatments together. While I am on only one medication right now which is Otezla it has caused me to lose some weight. In my case I was glad for it. If you are apprehensive about it please have a very detailed discussion with your doctor. I wish you nothing but the best. Vickie W., Team Member

      2. I was ready to try Xeljanz a year ago, right around the time they added a stronger black box warning to it. I then decided it was too risky, so I went with Taltz. I actually ended up being intolerant to Taltz...it created a strep throat-like condition in me every time I injected it and lasted for 7 days each time. I had good results trying Otezla with a biologic, but my insurance refused to cover it plus my biologic. The Otezla manufacturer was unwilling to reduce the cost or provide me with any type of financial assistance. Currently, I have failed Simponi and am returning to Enbrel. Enbrel has helped the most of all the biologics I have tried so far. It isn't great at helping my symptoms, but it does seem to be a wee bit better than the others.

    3. Hi, I have mainly been on TNF blockers for the last 12 or 13 years and they work moderately well for me. My rheumatologist switched me to Stelara (Il12/23) a couple years ago and I got much worse and went back to Cimzia. He switched me again to Tremfya (Il23) 18 months ago and Again I got much worse and switched to Enbrel another TNF. TNF blockers seem to work best for me but I know others have different experiences. It’s hard to switch but I keep hoping to finally find the perfect biologic. I pray you find the one that works best for you.
